As a Supported Living Coordinator (SLC) you will:
- Assist with all aspects of supervision for assigned caseload including hiring, disciplinary action, and development
- Work with support staff to ensure service implementation as stated in Individual Service Plans. Attend staff meetings and/or provide on-site training when needed
- Coordinate development of Individual Service Plans and update key documents to reflect service need changes within established timelines
- Collaborate with the Compliance and Quality Improvement (CQI) department to ensure recommendations are implemented in a timely manner
- Act as the main contact for stakeholders
- Work with internal and external parties to facilitate referral process
- Collaborate with Finance Department to develop individual and site budgets that reflect services outlined in Individual Service Plans
The applicant should have experience in the following areas:
- BA/BS in a human services related field from an accredited college or university preferred
- Experience working with people with disabilities and implementing service provision
- Experience supervising multiple employees
- Applicant must maintain a valid driver’s license and a driving record that is acceptable from a risk management perspective
