PEDIATRIC INTEGRATED HEALTH (PIH)
Tanager Place Pediatric Integrated Health (PIH) coordinates services for children ages 4-18 who are a Medicaid recipient and have a qualifying behavior health diagnosis. A professional care team helps to coordinate and streamline medical and behavioral health services to ensure that children’s whole health and wellness needs are being met.
KEY RESPONSIBILITIES:
To function as a member of the Pediatric Integrated Health Care Team providing coordination of services to families on the Children’s Mental Health Waiver and Habilitation Programs. Treat all individuals with respect and maintain standards of confidentiality, professionalism and objectivity. Provide linkage, follow-up and support of behavioral, physical and supportive care for PIH-ICM client/family. This requires the ability to:
QUALIFICATIONS:
A BA/BS in the human services field and a minimum of two years of experience working directly with children with mental health and/or physical health needs. This position requires strong communication skills, both written and verbal, as well as the ability to relate to various family dynamics, which will include crisis management skills. The ideal candidate will be a self-starter, and have excellent organizational and time-management skills. Knowledge and understanding of Managed Care Organizations, Children’s Mental Health Waiver and Habilitations services are preferred. Schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m.; flexibility required.

- Understand Child Mental Health Waiver and Habilitation services.
- Have a base knowledge of Serious Emotional Disturbance (SED) diagnoses and treatment modalities.
- Effectively assess the needs of client/family.
- Develop and maintain healthy, supportive relationships with client/family.
- Locate, coordinate and link client/family to needed behavioral, physical and social services in the community.
- Complete all necessary paperwork, including Individual Care Plans and InterRAI and work in various computer systems.
