Care Initiatives - Hospice is currently seeking a Weekend Admissions Nurse to join our team!

Hours: 10-6 p.m.

The Admissions Nurse is responsible for facilitating patient transitions of care during admission which includes coordination with Central Intake, Clinical Manager, Administrator or Community Representative. A qualified admissions nurse is a licensed RN that will provide visit support to new admissions under our hospice care.

Our hospice caregivers are uniquely qualified to attend to the needs of patients in the final stages of life. We meet the patient and family members where they are, going to their home, a long-term care center assisted living residence or hospital to administer compassionate hospice care.

Our approach addresses the physical, emotional, and spiritual needs of those facing life-limiting illness for themselves and their families. We work closely with family members and caregivers to respect the wishes and needs of our patients and their loved ones with the utmost compassion, dignity, and respect for those in their final stages of life.

Responsibilities:

  • Provide nursing services to Care Initiatives Hospice patients, through visits that may include telephonic and in-person management of patients, families, caregivers and referral or community sources.
  • Responsible to establishes positive relationships with community sources with the intent to enhance and expedite patient admissions and coordination of care, medications, equipment, and supplies.
  • The Admissions nurse determines immediate needs and initial assessment, shares findings with the RNCM and collaborates care with the IDT.
  • Provides visits to assess ongoing patients’ needs, revising care plan as needed and communicating patient assessments to the team, when not doing admissions.
  • Provides clinical services to patients and families that address goals of the care plan.
  • Prepares clinical and progress notes; maintains and responsible for the clinical record; coordinates services and informs physician and other personnel of changes in patient condition and needs.
  • Communicates with staff and necessary resources to coordinate services for patients and families.

Qualifications:

  • Current, valid Iowa registered nurse license or license in a compact state
  • Current driver’s license and automobile liability insurance
  • CPR certification required before first day of work
  • Obtains Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
  • Current home access to the internet
