Memory Clinic Nurse Navigator

This position supports Mercy's philosophy of patient centered care by coordinating the care of individuals concerned about changing cognitive abilities.  The Memory Clinic Nurse Navigator  expedites an evaluation to determine whether a concern about changing cognitive abilities is dementia, a medical condition other than dementia, or a normal part of aging.  The Memory Care Nurse Navigator works closely and collaboratively with other professionals also focused on supporting individuals and families concerned about changing cognitive abilities.
Job Duties
  • Participates in ongoing educational opportunities. 
  • Provides education at Mercy Medical Center and in the community. 
  • Attends clinic department meetings. 
  • Engages the social worker to assist families needing Mercy Medical Center or community resources. 
  • Coordinates with social worker when home visits are needed because it is difficult for patients and/or care partner(s) to come to the Clinic. 
  • Follows up with care partners and is available to answer questions (this could include nights and weekends). 
  • Records patient and care partner(s) concerns and goals in medical record. 
  • Screens patients and care partner(s) (if applicable) for medical issues, anxiety and depression. 
  • Gathers social and medical histories. 
  • Helps facilitate referral appointments. 
  • Serves as the last point-of-contact with patients and care partner(s) the day of the clinic visit (sometimes with the social worker) to discuss:
    • Clinic results
    • Provide educational materials specific to the diagnosis
    • Process the information given
    • Begin creation of an individualized plan/notebook for the patient and care partner(s)
  • Serves as the first point-of-contact with patients and care partner(s) upon appointment being scheduled. 
  • Participates as a member of a multi-disciplinary team determining whether a concern about changing cognitive abilities is dementia, a medical condition, or a normal part of aging. 
Department Specific Duties
Knowledge, Skills and Abilities
  • Demonstrated flexibility and willingness to work as a member of a team
  • Energy and interest in working with people living with dementia and their care partners and families
  • Interest in continual learning and desire to make improvements
Professional Experience
  • Minimum five years direct experience in chronic care nursing required. 
  • Three years clinical or indirect experience in memory nursing preferred. 
  • Experience in dementia preferred. 
  • Caregiving experience preferred. 
  • Bachelor of Science Nursing (BSN) preferred. 
  • Willing to obtain dementia education required. DAWN certification required within the first six months upon hire. 
Licensure, Certification, Registration
  • Must be licensed as a Registered Nurse in the state of Iowa.
  • Iowa Dependent Adult Abuse Mandatory Reporter training required.  Employee must submit certification of completed training to Human Resources prior to start or transfer date.
  • Current valid driver's license, own reliable transportation and automobile insurance.
Physical Demand Requirements
  • Light: Exert up to 20 lbs. of force occasionally and/or up to 10 lbs. of force frequently

Schedule: Full Time, Day, 8:30am to 5pm M-F
