Health Coach, Iowa Return to Community, RN preferred

$27.65 per hour.  Additional income between $5,154-$6,348/per year on top of wage depending on the health plan selected.

GENERAL JOB SUMMARY:

Under supervision of the Executive Director and the Integrated Services Director, the IRTC Health Coach provides client centered services to older adults to ensure a safe transition to return into the community from a hospital/skilled nursing facility and facilitates communication between professional staff in the hospital or nursing home and the patient and/or family. This includes but not limited to following up on appointments, personalized health information and disease education.

The Health Coach manages multiple resources, assists the consumer to streamline access to appropriate and necessary services and ensures the person’s health, safety and welfare upon return to home and community.

The Health Coach will work in a team environment to support the Heritage care continuum offered through effective coordination, support and appropriate referrals. Assistance is provided to individuals in person, in their home setting, office setting, virtually and over the phone. 

 

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Complete Outreach/Marketing activities as necessary and assigned for the IRTC program; serve as the “face” of the program.
  • Utilizing WellSky, complete and maintain documentation, both paper and electronic, in a timely manner as required by state and federal regulations, program and other funders.
  • Collaborate with hospital and/or nursing home discharge staff to identify appropriate patients for care transitions services.
  • Visit referrals in the nursing home and/or hospital pre-discharge
  • Serve as the bridge between the discharge staff in the nursing home and/or hospital and the patient and/or family
  • Serve as a guide to the patient, coaching the patient in addressing critical issues and self-management tasks rather than directly taking over and providing care, using techniques including over-the-phone and in-home assessment, planning, referral and linkage
  • Formulates an individualized plan in collaboration with the consumer, family and medical staff, considering and assessing the consumer’s condition, diagnosis, medications and support systems.
  • Promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence
  • Follows up after discharge
  • Makes follow-up phone calls to monitor and evaluate success in accordance with program requirements and guidelines.
  • Collects and analyzes data to identify opportunities for improvement.  Develops, prepares and presents related records and reports.  Participates in measuring outcomes, data procurement and analysis activities.
  • Serves as an ongoing advocate on behalf of IRTC
  • Participates in quality assurance activities for the program
  • Develops and maintain relationships with key individuals, medical providers, and entities in the community
  • Performs other related work duties as assigned

Working Conditions

Position involves working in an office setting. Occasional (10-33%) standing, pushing, pulling and grasping. Frequent (34-66%) reaching and viewing a computer and other types of close visual work.  Constant (67-100%) sitting, walking, repetitive motion, talking and hearing.

MINIMUM QUALIFICATIONS FOR EDUCATION AND EXPERIENCE:

 

  • Bachelor’s degree in a human services field (preferred one year of experience); OR
  • License to practice as a registered nurse (preferred one year of experience); OR
  • License to practice as a licensed practical nurse and four years of experience working in the areas of aging, disabilities, community health or hospital discharge planning; OR
  • Bachelor’s degree and two years of experience working in the areas of aging, disabilities, community health or hospital discharge planning; OR
  • Associate’s degree and four years of experience working in the areas of aging, disabilities, community health or hospital discharge planning;

 

 LICENSES, CERTIFICATIONS, OR REGISTRATIONS REQUIRED:

  • Must have a valid driver’s license from the state of Iowa and vehicle with valid insurance for required travel.
  • Become certified as a Dependent Adult Abuse Mandatory Report within 30 days of being hired.

PREFERRED QUALIFICATIONS, LICENSES, CERTIFICATIONS, OR REGISTRATIONS:

  • Experience with disability and/or aging services and with discharge planning in a medical setting preferred.
  • LPN or RN is preferred.
