Position Summary:
To manage the daily operations and supervision of staff to ensure quality programming and achievement of established Pediatric Integrated Health (PIH) program outcomes. Including ensuring all staff function as members of the Pediatric Integrated Health (PIH) Care Team, providing direct and indirect coordination and education regarding health and wellness for clients, and managing the Intensive Case Management (ICM) Team that oversee the Children’s Mental Health Waiver (CMHW) Program and Habilitation Services (HAB).  

Examples of Essential Functions:
  • Provide supervision, leadership, direction, and assign work/tasks to the PIH and ICM team;
  • Monitor and review documentation for content and timely completion;
  • Provide quality care and delivery of waiver services;
  • Ensure CMHW  and HAB service delivery standards are met;
  • Provide support and facilitate training to grow PIH and ICM employee’s professional skill sets;
  • Maintain ongoing knowledge regarding state regulations and updates;
  • Treat all individuals with respect and maintain standards of confidentiality, professionalism and objectivity;
  • Assess the physical and mental health needs of clients;
  • Understand and implement population management strategies;
  • Ensure necessary assessments and surveys are completed timely;
  • Provide leadership and support for the development of health and wellness program components and activities;
  • Provide education to program staff to support client interventions and staff competency for health and wellness initiatives;
  • Oversee the implementation and support of physical wellness interventions/activities for identified client cases;
  • Ensure regular telephonic communication with families to support on-going engagement and pro-active care management services are taking place;
  • Attend staffing’s, case reviews and service delivery meetings as needed;
  • Serve as a liaison with medical care providers;
  • Utilize technology and information systems as directed;
  • Complete file reviews monthly;
  • Maintain required documentation and files and utilize available data and information to effectively monitor and improve program performance; 
  • Utilize technology and information system as directed; including TAV, ISIS, IMPA, MCO portals, and an electronic health record;
  • Be familiar with policies and procedures available in agency orientation, treatment and personnel manuals;
  • Understand consumer service concepts and initiatives.  Demonstrate these skills on a regular basis with both internal and external consumers;
  • Participate in the Quality Improvement process;
  • Attend regularly scheduled department, employee, and other assigned meetings;
  • Ensure staff understand consumer service standards, are consumer focused;
  • Serve as a positive consumer service role model for staff;
  • Pursue professional development through attending conferences, workshops and seminars;
  • Demonstrate appropriate age-specific and cultural competencies in interactions with clients, families, co-workers and other professionals;
  • Be aware and respectful of cultural and socioeconomic characteristics of clients/family;
  • Completes all necessary trainings as assigned including required continuous education, conference attendance, review of relevant materials, and completion of   Relias trainings;
  • Performs all work duties and activities in accordance with Tanager Place policies; procedures and safety practices;
  • Attends work regularly at the designated place and time;
  • Performs related work as required.
  • Effectively utilize the agency’s Electronic Health Record system;
  • Knowledge and use of the agency’s Resilience and Trauma-Informed Care model;
  • Proficient use of Microsoft Excel, Outlook, and Word or similar software;
  • Proficient typing skills;
  • Ability to establish and maintain complex records and files;
  • Ability to handle confidential information with tact and discretion;
  • Ability to use empathetic listening skills, communicate with clarity and maintain an attitude that conveys respect;
  • Ability to operate a personal computer using program applications appropriate to assigned duties and responsibilities;
  • Ability to work cooperatively and to maintain effective working relationships to accomplish job responsibilities;
  • Ability to quickly learn and put to use new skills and knowledge brought about by rapidly changing information and/or technology;
  • Thorough knowledge and experience as a productive team member with the flexibility to adapt to daily changes in routine or assignments;
  • Ability to critically think through issues/problems and identify effective solutions/outcomes;
  • Ability to communicate effectively and establish and maintain positive effective working relationships with other Tanager Place employees, state and federal stakeholders, program participants, and the community;
  • Ingenuity and inventiveness in the performance of assigned tasks;
  • Communicate effectively in writing and verbally;
  • Strong written and verbal communication skills, as well as crisis management skills, necessary;
  • Ability to act as a positive emissary of Tanager Place in all meetings with public and other agency employees to promote positive image of Tanager Place at all times.

 Acceptable Experience/Training/ Education:
  • Bachelor’s in human services or a related field and a minimum of three years of supervision experience, including hiring, training and supervising numerous team members.  
    1. Experience of team and client engagement required.  Familiarity with the Medicaid system and area resources strongly recommended;
  • Any equivalent combination of experience and training which provides the knowledge and abilities necessary to perform the work.
