Position Title: Permanent Supportive Housing Case Manager – Fairweather Lodge

Position Summary:  The Fairweather Lodge Case Manager is responsible for the day-to-day implementation of the Lodge training program, Lodge member houses and Supported Community Living Services for Lodge clients.  This position is responsible for assisting tenants in maintaining housing, direct service, and coordination of services within the agency and with other service providers.

Reports To: Mental Health Recovery Manager

Key Responsibilities:
  1. Lodge participants as they progress through the training program by providing services such as medication management support, emotional support, transportation, supervision, and basic living skill instruction.
  2. Implement and facilitate curriculum to train and educate trainees on social, health, mental health, vocational, and basic living skills.
  3. Process program applications. 
  4. Attend weekly team meetings to review tenant situations, concerns and issues.
  5. Provide direct care including, but not limited to, independent living skills, employment assistance, and transportation, for all tenants in Fairweather Lodge 
  6. Recruit, train, and support volunteers to assist in the delivery of the Fairweather Lodge program.
  7. Provide essential SCL program services to the caseload, including admissions, assessments, treatment planning, and discharges.
  8. Maintain all required documentation.
  9. Review and complete incident reports as required.
  10. Ensure all services hours are billable according to Medicaid requirements.
Other Responsibilities: 
  1. Ensure that tenant rights are protected. 
  2. Participate in Quality Improvement activities including data collection and analysis. 
  3. Develop and maintain knowledge of resources, services and opportunities available to tenants. 
  4. Carry a cell phone at all times when on duty or when on-call. Respond to issues in a timely and professional manner. 
  5. Other duties as assigned.
Position Qualifications:

  1. Knowledge, skills and abilities consistent with Bachelor’s degree in in human services related field, or minimum three years related experience.
  2. Minimum of' one year direct care experience with persons with mental illness or developmental disabilities.
  3. Ability to act responsibly in a crisis and use good judgment.
  4. Proficiency in Microsoft PC and Windows applications.
  5. Good written, verbal, and interpersonal communication skills.
  6. Possess a valid driver's license and reliable transportation to get to and from work shifts.
  7. Possess working knowledge of state and federal regulations pertaining to HCBS (e.g. Iowa Medicaid, Iowa Code, DHS) or ability to quickly acquire this knowledge.
Salary and Compensation: Commensurate with experience. PSH Case Manager is a full-time position with health, dental, paid leave and retirement plan benefits.
