Pediatric Integrated Health Care Coordinator

Children with a psychiatric diagnosis and their families receive coordinated services from our professional care team. Our team helps families understand the available services and works to identify the most beneficial support for clients and families.
PIH helps clients and their families:
  • Find a doctor, dentist or counselor
  • Manage medications and prescriptions
  • Provide advocacy and 24/7 support
  • Obtain education and wellness support
  • Serve as a resource for mental, behavioral and physical challenges
  • Answer questions and look for new services
As a Care Coordinator you will provide direct and indirect coordination of services to client/family.
To function as a member of the Pediatric Integrated Health (PIH) Care Team providing direct and indirect coordination services to client/family. In addition, this position supports transitional living processes for client, provides linkage, follow-up and support of behavioral, physical, and supportive care for PIH client and families.

  • This position requires a BA/BS in human services field and a minimum of two years’ experience working directly with children with mental health and/or physical health needs.
  • Experience with Managed Care Organizations (MCOs) and Medicaid a plus.

Applicants must have a proven work history of strong organizational, time management, and decision-making skills. Strong written and verbal communication skills are necessary to ensure the timely and accurate exchange of information within family and team members. The ability to provide interventions to clients and their families while in crisis is highly recommended. This position will provide direct and indirect support to families who have a child with a serious emotional disturbance. Focus on empowering caregivers, providing necessary education regarding behavioral and physical health care and to assist consumers in locating and accessing social/community and recovery supports. Will work collectively with a care team of a nurse and family support worker to coordinate physical and behavioral healthcare services. Both face to face and telephonic support to families is expected to assist families in meeting the goals they have identified. Rotating on-call duties quarterly. Reliable transportation is necessary.
