Supported Living Coordinator - Enhanced

Job Information:

Position & Assignment: Supported Community Living.  Majority of sites on caseload are Enhanced Behavioral locations.

Minimum Qualifications:  Preference of a BA/ BS from an accredited college or university and/or a combination of education and experience working with people with disabilities which totals five years. Two years of prior supervisory experience required. Experience in service provision is essential.
Other Qualifications:  A valid driver's license and a driving record that is acceptable from a risk management perspective are required. The physical ability to assist in lifting individuals may also be required for some assignments. The ability to communicate clearly in oral and written form is essential.
Position Summary:  The Supported Living Coordinator is responsible for mobilizing team members to ensure services are provided as stated in the individual service plan. Supported Living Coordinators utilize the strengths of each employee and delegate accordingly. Responsibility for those duties being performed as required by the code of the state of Iowa, contractual provisions with funding authorities, and the policies and procedures of the agency remains with the Coordinator.

Primary Responsibilities:
A.  Service Provision
  • Works with support staff to ensure service implementation as stated in Individual Service Plan. Attends staff meetings and/or provides on-site training when needed.
  • Coordinates development of Individual Service Plans and updates key documents to reflect service need changes within established timelines.
  • Performs monthly face-to-face meetings with individuals at their homes to ensure appropriate services.
  • Provides coaching, problem-solving and mediation for individuals and their ISP teams to resolve conflicts and foster relationships relative to the needs of the individual.
  • Collaborates with the Compliance and Quality Improvement (CQI) department to ensure recommendations are implemented in a timely manner.
  • Acts as the main contact for stakeholders.
  • Works with internal and external parties to facilitate referral process.
B.  Supervision
  • Trains Supervisors using established agency training tools within designated timelines. Ensures staff adhere to necessary training guidelines set forth by the CQI department.
  • Schedules and facilitates 1:1 meetings with Supervisors on a regular basis. Uses meetings to train, problem-solve, and set priorities.
  • Completes and delivers direct report evaluations within established timelines. Provides on-going, timely feedback on performance and addresses performance concerns.
  • Assists with all aspects of supervision for assigned caseload including hiring, disciplinary action, and development.
  • Completes or delegates all necessary job duties in the event of a Supervisor vacancy, which may include working direct care shifts.

C.  Financial
  • Reviews site schedules and budgets with Supported Living Supervisor to ensure service provision remains within established budgets.
  • Collaborate with Finance Department to develop individual and site budgets that reflect services outlined in Individual Service Plans.
  • Resolves financial and budgetary concerns relating to assigned caseload.
  • Completes necessary payroll forms within designated timelines.
  • Reviews and approves Supported Living Supervisor timesheets in accordance with Finance Department guidelines.
D. Secondary Responsibilities
  • Participates in the on-call rotation of designated cluster group.

  • Certain position requirements occur in individual homes. The ability to travel extensively throughout the community is required
  • This position requires communication and interplay with a number of internal and external stakeholders. Therefore, professional and timely communication is considered a key component of this position.

This is a full time salaried exempt position with evening and weekend hours.
