Supported Community Living Service Coordinator

Job Information:

Description: The SCL Service Coordinator is a full-time position responsible for the day-to-day implementation of SCL services for the individuals on their caseload and enrollment of new clients. The Service Coordinator is responsible for direct service, coordination of services within the agency, with county case management, and with other service providers to expand the agencies SCL program.

Key Responsibilities:

  1. Ensure that individual health, safety, and welfare needs are met.
  2. Provide essential SCL program services to your caseload, including admissions, assessments, treatment planning, and discharges.
  3. Provide direct care including transportation for individuals in the SCL program.
  4. Coordinate and monitor designated individual services.
  5. Each person’s Individual Service Plan is being implemented according to state regulations and progress is being made and documented.
  6. Build and maintain positive professional relationships with the individuals, their families/guardian, and with referral sources.
  7. Review and complete incident reports as required.
  8. Ensure all service hours are billable according to Medicaid requirements

Other Responsibilities:

  1. Ensure that individual rights are protected.
  2. All files and paperwork are completed to standard. This includes but is not limited to intake paperwork, social history, assessment, ISP, services document notes, quarterly reviews, etc..
  3. All individual files comply with state regulations.
  4. Appointments and meetings are coordinated as needed to ensure that quality services are provided and individuals’ needs are met.
  5. Follow-up is completed with regard to medical appointments/concerns and any other individual concerns.
  6. Case notes are maintained and include any pertinent changes/info (e.g. collateral contact, medical, face-to-face).
  7. Carry a cell phone at all times when on duty or when on-call. Respond to issues in a timely and professional manner.
  8. Other duties as assigned.

Position Qualifications:

  1. Knowledge, skills and abilities consistent with a Bachelor’s degree in human services related field, or minimum one year related experience.
  2. Minimum of' one year direct care experience with persons with mental illness or developmental disabilities.
  3. Proficiency in Microsoft PC and Windows applications.
  4. Good written, verbal, and interpersonal communication skills.
  5. Possess a valid driver's license and reliable transportation to get to and from work shifts.
  6. Possess working knowledge of state and federal regulations pertaining to HCBS (e.g. Iowa Medicaid, Iowa Code, DHS) or ability to quickly acquire this knowledge.
