Care Initiatives is currently seeking a Social Worker to join our team!
Our Hospice Social Workers will be an important resource for patients and families who are processing the approaching loss, helping them to navigate the emotions that come with it in collaboration with an interdisciplinary team.
Our approach addresses the physical, emotional, and spiritual needs of those facing life-limiting illness for themselves and their families. We work closely with family members and caregivers to respect the wishes and needs of patients and their loved ones with the utmost compassion, dignity, and respect for those in their final months of life.
Purpose of the position:
The Bachelors prepared social worker provides support to Care Initiatives Hospice patients, families, and/or caregivers, and works closely with the interdisciplinary team to ensure the quality of life and patient-focused individualized care.
The primary responsibilities are to complete the comprehensive psychosocial assessment/evaluation of patients and families. Evaluate patient/family/caregiver's emotional, social, financial, and environmental resources. Identify appropriate psychosocial problems, interventions, and goals for care plans. Participate in the development and implementation of initial and comprehensive care plans. Regularly re-evaluates patient and family needs revising care plan as needed and communicating assessments to the interdisciplinary team (IDT).
Qualifications:
- Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education and is supervised by an MSW; or
- Baccalaureate degree in psychology, sociology or other field related to social work and is supervised by an MSW and has one year of social work experience in a health care setting.
- Current Iowa Driver’s license and automobile liability insurance.
- Knowledge of end-of-life care related to social services.
- Two years experience in working with terminally ill persons and their families preferred.
- Ability to access businesses and patient homes.
- Ability to establish community relationships, utilize community resources.
- Ability to plan, organize, and effectively communicate both orally and in writing.
