This position supports Mercy's philosophy of patient centered care by assisting in the rehabilitation of cardiac and pulmonary rehab patients through exercise and education
 
Job Duties
  • Ensure that all accurate and clinically relevant information is communicated with patients, their families and co-workers at hand offs and other times as needed. Take all measures necessary to ensure that the appropriate and involved parties have communicated necessary information in order to allow for the best possible patient outcome.
  • Assesses and evaluates new patients at intake into program
  • Conducts orientation session with patient, which includes interpretation of exercise prescription, guidelines for safety, and overview of program.
  • Assists participant in goal-setting during the initial evaluation and throughout the program/patient stay, and participates in discharge planning.
  • Assists in determining progression of exercise program
  • Conducts exercise therapy session in a safe professional manner.
  • Provides selected classes in patient and family education in areas of expertise.
  • Competent with patient assessment and plan of care including consideration of biophysical, psychosocial, spiritual, environmental, self care, educational, and discharge planning.
  • Demonstrates competency in implementing the patient's plan of care, including writing exercise prescriptions, calculating target heart rate, and educational sessions.
  • Demonstrates clinical competency and practices within established standards of the American College of Sports Medicine and American Association of Cardiac and Pulmonary Rehab
  • Demonstrates competency in responding to emergencies and life threatening situations, including management of angina, and arrhythmias within the scope of the ES practice.
  • Performs Heart to Heart evaluations including assessment, education and follow up with patients as appropriate
  • Schedules for Heart to Heart program, maintains outcomes data and presents as needed.
  • Performs 12-lead EKGs and associated process including monitoring and follow up of critical values.
  • Performs event monitor and holter monitor placement with appropriate paperwork.

Knowledge, Skills and Abilities
  • Sound background in exercise and health promotion principles and procedures. Interest in cardiac/pulmonary rehab, cardiovascular and pulmonary diseases and educational strategies for CAD and Pulmonary risk management.
Professional Experience
  • Working knowledge of principles of cardiac and pulmonary disease processes including heart rhythms, adult education, health education, and wellness concepts preferred.
Education
  • Bachelor's Degree in Allied Health Field with course work specific to basic exercise science, health promotion, and health risk appraisal techniques highly preferred.
Licensure, Certification, Registration
  • Current certification in Basic Life Support (BLS), in accordance with the American Heart Association required.
  • Advanced Cardiac Life Support (ACLS) required or attainable within 6 months of hire.
  • American College of Sports Medicine (ACSM) certification as Exercise Specialist preferred.
  • Iowa Dependent Adult Abuse Mandatory Reporter training required. Employee must submit certification of completed training to Human Resources prior to start or transfer date.
Physical Demand Requirements
  • Light: Exert up to 20 lbs. of force occasionally and/or up to 10 lbs. of force frequently
