The LPN is responsible for the delivery of safe and reliable patient care, in coordination with the healthcare team. The primary role of the LPN is the execution of the nursing process in providing required care and evaluation of the care given. Responsibilities include: assessment, nursing diagnosis related to focus problem areas, planning, implementation and evaluation of nursing care. Performance of these responsibilities requires specialized knowledge, critical thinking and skills based on principles of psychological, social, physical and biological sciences that utilize the nursing process.


Clinical Excellence/Patient Care

·Assesses the needs of patients and families, providing appropriate teaching and making the necessary referrals to provide continuity of care.

·Develops an individual plan of care using a collaborative approach with the patient/family and/or health care team.

·Promptly completes documentation to assure an accurate legal record of patient’s care.

·Delivers high quality care demonstrating knowledge and clinical skills in accordance with established policies, procedures, and standards of care.

·Utilizes holistic approach (considering biopsychosocial, spiritual, educational, safety, and related criteria) appropriate to the age of the patients and families served in the assigned service area.

·Delegates and provides direction to other co-workers, as appropriate, and holds them accountable for their involvement in the plan of care.

·Utilizes critical thinking in making independent judgments and maintains responsibility and accountability for the knowledge of conditions of assigned patients.

·Demonstrates a working knowledge of aseptic technique and adheres to the essential functions and responsibilities set forth-as applicable

·Licensed Practical Nurse must be able to make independent judgment based upon knowledge of the overall departmental functions, policies, and procedures

·Licensed Practical Nurse carries out regular and special assignments to completion on the basis of general instructions and with a minimum of supervision and guidance.The Licensed Practical Nurse is organized and efficient in performing required nursing activities and has the ability to work independently within center guidelines to meet the needs of our customers.

·Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.Takes appropriate action on concerns reported by department staff related to compliance.

Minimum Requirements


Preferred or Specialized




Successful completion from an accredited licensed practical nursing program




Minimum of 1-year experience in a clinic setting




Licensure in good standing to practice as a Licensed Practical Nurse in the state of Iowa.

BLS required







Writes, reads, comprehends, and speaks fluent English


Multicultural sensitivity


Critical thinking skills using independent judgment in making decisions


Basic computer knowledge using Microsoft Office, web browser, and email





Use of usual and customary equipment used to perform essential functions of the position
