Meth-Wick Community

The Meth-Wick Community is hiring a full-time social worker. This position will be responsible for the following:

Social Work Services to Independent and Supported Independent Living Residents.
a.    Make periodic assessments of residents referred by Level of Care team to ensure that all residents are receiving the appropriate services and are at the appropriate level of care.
b.    Provide information on the level of care process to residents and families.
c.    Help ensure the safety of all residents by serving on the Level of Care team.
d.    Assist residents and their families in coping with level of care changes by providing illness and adjustment to life changes counseling, referral to health care agencies, case management service and discharge planning.
e.    Responsible for pre-admission assessments and social histories of applicants to Independent Living and Assisted Living to ensure placement at the appropriate level.
f.    Completes residents PASRR assessments for residents transferring from Independent Living or Assisted Living to the Nursing Facility.

Responsible for Admissions/Sales of Custom Care and Arbor Place.
a.    Conduct tours for all inquiries to assisted living as needed.
b.    Complete all admission paperwork and living unit agreements.

Responsible for Social Work Services to Custom Care Residents.
a.    Develop initial care plan and provide quarterly reassessment of residents and assessments as needed.
b.    Provide social work services as outline in 1d.
c.    Assure that compliance with appropriate standards of care for Assisted Living and Independent Living residents are being met.

Responsible for Social Work Services to Arbor Place Residents.
a.    Provide pre-admission testing and assessment of potential residents and respite care residents.
b.    Develop initial care plan and provide quarterly reassessment of residents and assessments as needed.
c.    Provide social work services as outlined in 1d.

Meth-Wick provides a competitive salary, benefits and a great place to work! The successful candidate must possess a Bachelor’s Degree in Social Work or a related field and preferably a minimum of two years’ experience in senior living.
