Johnson County Mental Health/ Disability Services’ Case Management program seeks a dedicated, organized individual to work with individuals with brain injuries and/or intellectual disabilities and their families to coordinate medical, social, educational, housing needs, transportation, vocational and other necessary services and supports.

Case Managers work with individuals and their families, service providers, and other agencies to assimilate services from multiple providers and funding streams into an integrated person-centered plan of care by monitoring and coordinating the support services necessary for individuals with disabilities to live healthy and safe lives at home and be a part of their community.

The ideal candidate will have strong organizational skills, attention to detail, flexibility, and the ability to manage multiple resources and tasks. Candidate shall also demonstrate effective decision-making skills, and strong written and verbal communication skills.

Minimum requirements:

  • Bachelor’s degree with 30 semester hours in a human services field (including, but not limited to, psychology, social work, mental health counseling, marriage and family therapy, nursing, education, occupational therapy, and recreational therapy) AND at least one (1) year of experience in the delivery of services to the population groups that the person is hired as a case manager to serve OR an Iowa license to practice as a registered nurse AND at least three (3) years of experience in the delivery of services to the population groups that the person is hired as a case manager to serve.

Special Requirements:

  • Must pass background investigation that satisfies Case Management accreditation approval.
  • Valid driver’s license and insurable under County liability coverage.
  • May require use of private vehicle or equivalent transportation for official county business.

$51,747.28 annually to start with outstanding health insurance and benefits.
