Under the direction of the Associate Director, the Health and Wellness Services Coordinator will coordinate, develop, monitor and provide technical support to Evidenced-based Programming, Health Promotion, Nutrition Education and other ancillary nutrition/wellness grants while working alongside the Regional Nutrition Coordinator and associated providers to ensure the sustainability of the nutrition and health promotion programs.

DUTIES/RESPONSIBILITIES:
  • Manage the day-to-day functions of health and wellness programs, evidenced based and health promotion programs and outreach.
  • Able to multi-task multiple projects and reporting at once.
  • Act as liaison between grant partners and volunteers.
  • Collaborate, develop and maintain relationships with key individuals and entities in the community, college/agency staff, clients, professionals, and volunteers.
  • Discover, recommend, and initiate innovative approaches for program maintenance, development, and enhancement through the Older Americans Act.
  • Provide general direction and oversight for all evidenced based prevention activities.
  • Maintain a record keeping system and information systems for the program.
  • Implement or participate in quality assurance activities for the programs.
  • Monitor the outcomes and program effectiveness, collect data, complete timely reports, and ensure that grant requirements are carried out
  • Train, educate and publicly speak on evidenced based classes and programming in Heritage.
  • Build upon existing infrastructure at both state and community levels expanding upon the number of participants in classes.
  • Develop and implement program specific strategic planning alongside management.
  • Assist in the development and implementation of program specific policies and procedures alongside management.
  • Provide training and educational outreach related to the program for college/agency staff and community members as assigned.
  • Keep current on program related information, initiatives, requirements, and resources.
  • Provide direct client services as assigned or as needed.
  • Act as back up to Encore Café Service and Volunteer Coordinator as needed.
  • Work with the Encore Café staff to ensure successful health programming.
  • Perform other related work duties as assigned.

  • Represent the college/ agency for agency, local, regional, and state committees, events, and meetings as assigned.

  • Be able to effectively utilize technology and computers including, but not limited to, office equipment, word processing, spreadsheets, and database software.

PHYSICAL ACTIVITIES WITH/WITHOUT REASONABLE ACCOMMODATION:

MINIMUM QUALIFICATIONS:
  • Bachelor's degree requiredin health,human service, nutrition, nursingor a related field.

LICENSES, CERTIFICATIONS, OR REGISTRATIONS REQUIRED:
  • Must have valid driver's license for required travel.

PREFERRED QUALIFICATIONS, LICENSES, CERTIFICATIONS, OR REGISTRATIONS:
  • Preference given to individuals with strong knowledge of health promotion and wellness activities geared to an older or disabled population.
  • Preference given to those with strong skills in program management.
  • Attend trainings to become leaders in evidenced based programs as deemed appropriate.
  • Must be willing to be certified in dependent adult abuse upon hire, if not already certified upon application.
