Position Highlights
The Admissions Coordinator will be responsible for the census growth, external marketing, and brand awareness, reviewing referrals to determine clinical appropriateness and state and federal criteria for SNF admission, talking to families and incoming residents to prepare them for the transition and set expectations, bed management, and partnerships between other departments to assure an excellent customer experience.

Responsibilities

  • Generating referrals
  • Converting referrals to admitted residents
  • Identify key referral partners & developing and maintaining relationships with referral partners
  • Creating a presence in the surrounding community by hosting and attending outreach events
  • Educating referral partners, referred patients, families, and surrounding community on services provided
  • Building relationships and trust with peers in the community
  • Creating a positive first impression of the building
  • Developing a relationship with the incoming resident and their family to help navigate them through this change
  • Act as a liaison between transferring institutions.
  • Greet and escort the new residents to the assigned nursing unit.
  • Assist with orientation of residents and families; explain residents’ rights and responsibilities; assist with adjustment to facility living arrangement.
  • Follow facility admission procedures and regulations and communicate resident admission or transfer to other departments, physicians, and ancillary service providers.
  • Receive, investigate, and resolve or report to higher authority concerns from residents/families in accordance with positive customer relations principles.
  • Observe all facility safety policies and procedures.
  • Maintain cooperation and coordination between nursing personnel and other facility departments in order to provide quality care, quality of life, and clean, safe comfortable surroundings for residents.
  • Perform other related duties as directed by his/her supervisor.


Qualifications
  • Bachelor’s degree in communications, marketing, nursing, health care administration, or other disciplines
  • Sales or marketing experience in long-term care or healthcare
  • Must be organized, high energy, able to prioritize, able to have multiple projects/tasks going at once, strong and effective communication
