Program Coordinator, Congestive Heart Failure

This position supports Mercy’s philosophy of patient centered care by assisting in CHF program development and monitoring. The program utilizes telehealth to coordinate closely with patients and their Mercy Cardiology provider. The CHF Coordinator presents excellent customer service and enables the facilitation of positive patient outcomes by developing and implementing written protocols and education of patients and nurses detailing the rapid communication and basic response to critical vital signs that is specifically required to minimize readmission risk and serious adverse health events. The CHF Coordinator will be expected to utilize and track metrics specific to Mercy’s patient population.
 
Job Duties
  • Follows Mercy's safety guidelines, carries out job-specific safety duties and responsibilities, and promptly reports any unsafe conditions, situations, incidents and injuries.
  • Documents protocols and procedures for program operations and trains staff on changes.
  • Develops and implements written protocol and education of nurses and other healthcare staff detailing the condition of CHF, signs and symptoms of deteriorating symptoms, modifiable risk factors, and patient education.
  • Responsible for arranging and leading educational initiatives around CHF for the Heart Center staff and patient populations.
  • Works closely with operational team, physician relations and marketing to promote program through multiple avenues.
  • Arranges and conducts patient education seminars in conjunction with providers
  • Coordinates patient care plans and executes physician orders and follow-up
  • Reports practice performance to the Clinic Operations Manager, identifying operational efficiencies or areas for improvement.
  • Assists in developing the documentation of pertinent patient information to provide quality outcomes, guided by Mercy Cardiology Providers.
  • Serves as the liaison between the facility and all cardiology providers.
  • Coordinates staff and patient education, outcome data collection and quality improvements efforts.
  • Maintains relevant policies and procedures.
  • Assists in development of program and implementation of clinical care pathways for the CHF patient population aligned with evidence-based practices

Knowledge, Skills and Abilities
  • Excellent communication and customer service skills, computer skills, organization, time management, attention to detail and the ability to remain poised and efficient in a fast-paced environment.
  • Must have the ability to bring together a multidisciplinary team to serve patients and build relationships. 
Professional Experience
  • Minimum 3 years specialty nursing experience required.  Cardiology nursing experience preferred.
Education
  • Bachelor's degree in Nursing or related field preferred.
Licensure, Certification, Registration
  • Current certification in Basic Life Support (BLS), in accordance with the American Heart Association required.
  • Currently licensed to practice as an RN in the state of Iowa or compact state required.
Physical Demand Requirements
  • Light: Exert up to 20 lbs. of force occasionally and/or up to 10 lbs. of force frequently
